TANK SHELL FAILURE

500,000 Gallon Slurry Tank
All America Services was contacted to assess a tank failure and design a repair. This was the failure of an improperly designed and installed access door in the slurry tank by a previous contractor who apparently did not fully understand storage tank design and construction. The ruptured 500,000 gallon tank flooded the adjacent city street with its contents. A large area of the tank shell plate was buckled and the foundation was severely damaged.

The damage led to additional consequences in the upper shell of this open-top tank. The stress forced the shell to bulge out of round.

In preparation of the repair and rehabilitation, workmen installed temporary shell reinforcement beams to support the weakened structure so that the buckled shell section could be safely removed.

Next, the shell, floor, foundation and embedded channel were all cut back beyond the damaged area, exposing the existing reinforcing bars and hooks.

In order to remove the damaged floor, the reinforcing bars were severed. To rectify this, new holes were drilled into the remaining concrete and new steel dowels were then epoxied in and tied to the existing bars.

A new 8-inch channel was rolled to match the existing channel, then positioned and welded to the remaining anchoring channel.

The remaining damaged shell and temporary scaffold was then removed in preparation for the newly fabricated shell plate installation.

|
Once the new plate was lifted into place, the temporary scaffold was reinstalled and the new plate secured to the shell.

With the shell secure, the temporary bracing was safely removed. Lastly, the new plate was fit into place, welded 100% and x-rayed to assure weld quality.
